Adjusting of Tech/Doc behavior

I really enjoy the new pharmacies in the game. Now for my problem:
At the start of the night shift people pile up in the pharmacy after being treated and the day shift tech is home already. It takes some time for the night shift to get there so I get warnings of patients about to leave the pharmacy.
Now, when my tech arrives, he serves exactly 1 patient and then goes off to have a meal and go to the restroom with 5 customers leaving angry. In reality, the tech would get fired for this.
So...either could we have persons arriving with needs already satisfied to a certain extent, so they do not leave their patients just after arriving or, even better: let each shift arrive 1 hour before it starts, so they can gather in the restroom get dressed, to the loo and help themselves with some food before their shift starts.
It is rather unlikely that someone arrives at 20:15 when shift starts at 20:00, just because he leaves home at shift begin, and then he obviously does not take his lunch break directly he arrives at work. And in between the patients sit in an empty hospital for about one hour until everything gets running again. Normally, the two shifts meet, have a talk what has been done and is to do or at least the shift supervisors do that and then they leave and not 'Oh, it is 20:00 - let us flee the workplace' with replacements coming at 20:15-20:30 and then have a break first.
