How Do Clinic Queues Work?

At the minute, I have a waiting room full of about 20 people, and 6 doctors in 'idle' status. They either have no patients on their doctor card, or the patients they do have are currently outside the labs, while other doctors have 5+ patients on their card and are calling patients.
