If by extra skills you mean specializations, yes they can over time.

Sorry, I didn't make it very clear.
workers have skill slots and special slots
I hope to train the skills and expertise of the workers.

Yes, they can get skills over time and you can decide what they do, but you can't train them. It will happen over time, experience is what makes good doctors...
