Zoning trauma room and some others

I cannot seem to custom zone a trauma room or observation room. I can only use the prefab to build what is there. I have selected the departments for room assignment/zoning but only gives the basics (doctor's office, corridor, waiting room etc)

Have you reached the part of the game where it unlocks the Hospitalization via the insurance objectives?

Did you also look at the right side of your screen ?
