How to update on GOG?

I don't use any auto-installer, I download my games manually and install. (Old slow PC so don't want any junk running in the background.)

So how do I update the game?
When new updates are released on GOG, we just get the full game in a newer version (logical since it's such a small game).
Should I run the installers and just install it over the old installation?
Or should I uninstall the game, plus Doctor Mode DLC, and make a new clean install?

And will I keep all settings and savegames and achievements?

Hi, thanks for the message! We generally use Galaxy for testing the GOG versions, but from the game's perspective installing over the current folder manually should be fine too.
