I believe this examination hails from the Medical Procedures Mod. For the 'Disease' Respiratory Checkup, the Symptom 'Respiratory Checkup Appointment' lacks the description text in tooltip stating !LOC! SYM_JM_PULMONOLOGIST_CHECKUP_APPOINTMENT_DESCRIPTION instead of understandable text. See image:
