There's an issue with the shift change. The staff for the new shift spawns on the edges of the map at the same moment that the current shift drops everything and starts heading home. But it takes the new staff about an hour to actually walk to their spots. Most of the  time this means that patients won't get breakfast, since breakfast is over before the nurses even get to their office. But it gets a lot more problematic if there's a patient collapsing or a Trauma patient and then all of a sudden it's shift's over, leave the patient on the table and just go home. The new shift takes an hour to get to their stations, in  the meantime the patient is just lying around dying.

I would suggest you move the new shift staff spawn time an hour back. They can go to their departments break rooms until their shift actually starts (that's how we do it in real life btw).
Also, you could (should) implement that the staff of the shift that is over stays until their replacement is actually there on station (whoever uses their terminal during the other shift). For the staff that consistently shows up an hour late. The staff that works overtime can get a mood penalty or overtime pay or both.

Of course janitors don't matter for this.

I totally agree to this. Even though I can't recall any cases where I have actually lost a patient due to shift change, I do recall many close ones. When trauma night shift leaves, ambulance brings in a new emergency case and day shift can be seen running from the edge of the map to the trauma center. This has happened quite few times and each time I'm holding my breath, will they make it or not? Also there has been few similiar ones with the ICU.

I've been messing around with the scheduling.xml and you cannot overlap shifts. You can schedule breakfast an hour earlier but I am not sure that actually does anything but display the times differently on the lower left of the screen. I did twice witness nurses flinging trays at two patients just before rushing out the door at the end of their shifts. Sometimes medicine is delivered to an empty bed and sometimes to one with a patient, or at least I think it is medicine. It might be a liquid diet. It is not a tray, that is all I am sure of. The poor patients have to resort to buying snacks to satisfy their hunger when they don't get fed, which is good for my bottom line but bad for the reputation of the hospital.

By the way, if you change the times of the clinic hours, the first patients who arrive may not find any clinic staff to treat them and you will lose them for the day.  Apparently the timing of those patients is determined other than by Schedules.xml.

I completely agree with your sentiment, however this post should probably go in the suggestions forum as it is not a bug.

I could never understand why my patients were always angry about not eating despite having a food cart (I started placing more vending machines). I have had an issue with losing a patient due to staff leaving before the new staff arrive. In real life the doctors and nurses for each shift have to meet and discuss or "hand-off" their patients. This is done at the nurses station or break room (common room). It's technically illegal (USA) to abandon a patient like they do in the game. Even if the next shift nurse is late, it's illegal to leave before they arrive and take over care. I like the idea of them arriving early and waiting in the common room if need be. I've started firing staff that show up late because being without staff for 2-3 hours is dangerous.

No it's clearly a bug because it directly causes behavior that is negatively impacting the game in an unintended manner.
