Hi, thanks for the message! I assume we're talking about the DLC campaign level where the second objective is indeed "treat 8 patients of controlled doctors at emergency in intern mode".

This objective is meant to try out the hardcore difficulty settings in Doctor mode, so selecting one of the three options (button on the top of the doctor mode panel) will switch you from normal mode to intern mode, then treat a few patients to finish the task. Hope this helps!
