[QUESTION] Amount of Patients coming in too low

Despite having prestige over 3 stars the insurance companies are cutting the number of patients they send. All is well the first day or two in sandbox mode, but suddenly the patient load is cut because of "low prestige" despite my prestige increasing if anything. Is this working as desired, do I need 4-star prestige to get the full load of patients? Or is something amiss, because when starting the insurance companies had no problem sending the full amount?
