Patient died even though equipment was right there...

OK, unfortunately, my save file is after a 25 yr old patient died of pleurisy even though the machine to help save his life was moved to multiple spots that were accessible. I even bought two machines because the patient next to him was also collapsing from the same ailment!
Another issue I'm having is that the game will shut down at random intervals. The game continues, but it minimizes the game window.
Here's a screenshot of right after the 25 yr old died and faded away (no pathology dept). The other patient has the machine and the test is listed, but it's grayed out.

Hello! Please note that the presence of equipment on site is just one of the factors, there are also other things affecting the result of treatment. Time is one of the most crucial factors therefore the layout of the hospital and the workload of the responsible personnel may play a major role. Next is the level of experience of the doctor, which may affect the length of the treatment and time till the treatment actually takes effect, and the specialization of the doctor. For example, a doctor with a specialization in anesthesiology will perform better than anyone else when trying to suppress a collapsed state.
Whenever a patient dies, you should be able to perform an autopsy. The condition is fully functional pathology dept. One of the things worth checking could be morgue capacity. As you've mentioned the save you possess was made after the patient's death but we can still take a look at the layout and search for possible bottlenecks or staff and workload issues. Possibly we can also take a look at the log file and search for the missing patient.
Output_log and save might also help us to investigate the crashes you have. Guide how to get them is located on our forum
